- 申請個人業務(還原密碼等)請提供學生證、教職員證或其他身分證明文件。未提供者不受理。
- 意見反映請詳細說明內容，必要時請附截圖，如反映內容意義不明、不具名等不做回應。

- 依據《個人資料保護法》(暨施行細則)，告知本表單蒐集、處理及利用個人資料之事項：本表單依據個資法蒐集、處理及利用您的個人資料包含識別個人姓名及電子郵件地址。上述個人資料之蒐集、處理及利用，將僅限本表單業務需要使用，並遵守個資法之規定妥善保護您的個人資訊。您亦可拒絕提供相關之個人資料，但若不提供完整資料，資訊處將無法接受本表單及後續作業。
- For inquiries regarding courses, academic affairs, or general administration, please contact the relevant department directly (⌨Administrative Units / ⌨Academic Units). The Office of Information Technology does not handle non-IT related matters.
- When requesting personal account services (e.g., password resets), please provide a Student ID, Faculty/Staff ID, or other valid identification. Requests without valid identification will not be processed.
- Please provide a detailed description of your feedback or issue, and attach screenshots if necessary. Inquiries that are unclear or anonymous will not receive a response.
