- 電子郵件信箱與校務資訊系統密碼還原可透過「忘記密碼」自行交互辦理，請參考 本網站 內說明操作。
- 申請個人業務（包含但不限於還原密碼等）請提供學生證、教職員證或其他身分證明文件。未提供者不受理。
- 依據《個人資料保護法》(暨施行細則)，告知本表單蒐集、處理及利用個人資料之事項：本表單依據個資法蒐集、處理及利用您的個人資料包含識別個人姓名及電子郵件地址。上述個人資料之蒐集、處理及利用，將僅限本表單業務需要使用，並遵守個資法之規定妥善保護您的個人資訊。您亦可拒絕提供相關之個人資料，但若不提供完整資料，資訊處將無法接受本表單及後續作業。

- If you need to reset the password for your email account or the University Information System, you may use the “Forgot Password” function to reset it yourself. Please refer to the instructions on this website and follow the steps provided.
- When requesting personal account services (e.g., password resets), please provide a Student ID, Faculty/Staff ID, or other valid identification. Requests without valid identification will not be processed.Please provide a detailed description of your feedback or issue, and attach screenshots if necessary. Inquiries that are unclear or anonymous will not receive a response.
